2.2 Compliance with Medicare or your private health insurance provider requirements.

• professionals involved in your health care such as your medical practitioner or general dentist; for the purpose of referral; or
• de-identified photographs, radiographs and X-rays for teaching purposes; or
• radiology clinics for the purpose of undertaking X-ray examinations; or
• pathology laboratories.
3. HK Kendall Periodontal Health & Implant Surgery will digitally transfer your records to those involved in your health care as outlined in [2.3]. If your records need to be transferred by post, a fee may be incurred to cover the cost of registered post.
4. HK Kendall Periodontics & Implants will take reasonable steps to protect your information.
